Wisdom Tooth Removal

A thoughtful start to wisdom tooth removal.

Wisdom tooth removal in Warren is one of the most common dental procedures we perform at Smile Brightly Dentistry, and for good reason. Most people don't have enough room in their mouths for a third set of molars, and when those teeth start coming in, they can cause real problems fast. Dr. Mitchell Sable and our team handle extractions with care and precision, making sure you're informed, comfortable, and well-prepared before we ever get started.

Why Wisdom Teeth Usually Need to Come Out

Wisdom teeth, also called third molars, typically emerge in your late teens or early twenties. Because most mouths are already full by that point, these teeth often don't have anywhere to go. That leads to a range of issues that can get worse the longer they're left alone.

Some of the most common reasons we recommend removal include:

  • Impaction, where the tooth is stuck beneath the gumline and can't fully erupt.
  • Crowding that pushes neighboring teeth out of alignment.
  • Repeated infections or gum inflammation around a partially erupted tooth.
  • Cysts or other complications developing around an impacted tooth.
  • Tooth decay in a wisdom tooth that's too difficult to clean properly.

Not every wisdom tooth needs to come out immediately, but we monitor them closely. If we can see a problem developing, addressing it sooner rather than later is almost always the better call.

How We Evaluate Your Wisdom Teeth

Before recommending extraction, we take a thorough look at what's going on. That includes X-rays to see the position, angle, and root development of your wisdom teeth, as well as a clinical exam to check for signs of infection, crowding, or gum issues.

From there, we give you a clear picture of what's happening and what we recommend. If removal is the right move, we explain exactly what the procedure involves, what to expect during recovery, and how we'll keep you comfortable throughout. No pressure, no rushing, just honest information so you can make a confident decision.

What the Procedure Involves

The procedure itself varies depending on whether the tooth has fully erupted or is impacted beneath the gum and bone. A fully erupted wisdom tooth is removed in the same way as any other tooth. Impacted teeth require a small incision in the gum and, sometimes, removal of a bit of bone to fully access the tooth.

Either way, you won't feel pain during the extraction. We use local anesthesia to numb the area completely, and sedation options are available if you want to be more relaxed during the procedure. Most straightforward extractions are completed in under an hour, and you'll be on your way home shortly after.

It's a good idea to arrange for someone to drive you home, especially if you're having sedation. Plan to take it easy for the rest of the day.

Recovery: The First Few Days

Recovery from wisdom tooth removal is manageable for most patients, especially when you follow the aftercare instructions we send you home with. The first 24 to 48 hours are typically the most uncomfortable, but swelling and soreness improve steadily from there.

Here's what helps the healing process go smoothly:

  • Bite gently on gauze for the first hour to help a clot form in the socket.
  • Apply an ice pack to your cheek in intervals during the first day to reduce swelling.
  • Stick to soft foods like yogurt, mashed potatoes, and smoothies for the first few days.
  • Avoid straws, smoking, and anything that creates suction, which can dislodge the clot.
  • Rinse gently with warm salt water starting the day after your procedure.
  • Take any prescribed medications as directed and don't skip doses.

Most patients are back to normal activities within a few days, though full healing of the socket takes a few weeks. We'll check in with you after the procedure to make sure everything is on track.

What Is Dry Socket?

Dry socket is one of the more common complications after an extraction. It happens when the blood clot that forms in the empty socket becomes dislodged or dissolves too early, exposing the underlying bone. It's painful and can significantly slow your recovery.

The good news is that it's largely preventable. Avoiding straws, not smoking, and eating soft foods in the days following your procedure are the main things that reduce your risk. If you do develop dry socket, contact us right away. We can treat it quickly and get you back on track.

Is It Normal to Be Nervous?

Absolutely, and you're not alone. Many patients come in anxious about the procedure, especially if it's their first extraction. We find that most people are surprised by how manageable it actually is. The anticipation tends to be worse than the experience itself.

If anxiety is a concern, talk to us before your appointment. We offer sedation options that can make the experience much easier to get through, and we're happy to answer any questions you have beforehand so there are no surprises on the day.
